Please Complete these Forms when Registering for an Activity

Here are some guidelines as you get ready for the activities season!

• Students need to register prior to the start of practice. 

• MSHSL requires everyone in a sport must have an up-to-date physical on file. 

  • Physicals are good for 2 years.  You can contact your local healthcare provider to schedule one.

• When registering please do not create another account.

  • If you forgot your username or password, you can reach out to Amber or contact RSchools directly and they can help you reset.
