Medication - Prescription or routine medication at school
Medications that are given daily or routinely at school need a provider signature and parent signature.

Medication over the counter
For over-the-counter medication, a parent must sign the permission form and bring medications in the original labeled bottle to be used on an as-needed basis at school.

A form that needs to be completed and signed by the parent/guardian to allow communication and exchange of information that may be necessary for accurately helping to manage and keep a student healthy and safe during the school day.
